FINANCIAL POLICY
Effective January 01, 2007
ATTENTION: Medicare, Insurance plans with deductibles and Patient Responsibility Rules, Insurances with Co-Pays
Every January 1, most insurances begin your deductible portion for the new year.
Your deductible is the portion you are obligated to pay before your insurer begins coverage for the calendar year. The deductible is separate from and above the usual co-pay or other amounts owed by the patient.
Effective January 1, 2007 we will be collecting charges due by the patient at the time of the visit according to this schedule.
DEDUCTIBLES: All commercial insurers (to include Medicare) $100.00 plus any applicable co-pay
EXCEPTIONS: If you can demonstrate with a notice from your Insurance that your deductible has already been met, this charge does not apply.
PATIENT RESPONSIBILITY AMOUNTS (after deductibles):
All applicable co-pays are due at the time of visit.
If your benefit plan requires you to pay a portion or all of your visit costs, you will be expected to pay your balance on the day of the visit.
If your exact coverage cannot be verified, you will pay a minimum charge of:
Initial Visit: $100.00
Follow up visit: $50.00
REFUNDS
Refunds of any overpayments will be issued within thirty days of notice of overpayment, when verified by insurer documents. We usually receive notification from the insurer of the benefits applied to your visit within 45 days after the visit, however you may receive that information first and we would welcome a copy from you to start the refund process.
PRESCRIPTION REFILL POLICY
Effective September 15, 2006
PLEASE REQUEST PRESCRIPTION REFILLS THROUGH YOUR PHARMACY
Requests made before noon will be processed the same day. Requests received after Noon will be processed for the following day.
Please DO NOT call repeatedly to ask about when your prescription will be completed.
We process all prescriptions electronically for your safety. There are certain classes of drugs that require a paper copy of the prescription be used. If that is the case, you must pick up the prescription in person and show identification if requested.
You may request a refill only for your own use. You may not, for example, have a friend call for you. A family member may call for a refill if those arrangements have been made ahead of time and are recorded in your chart as valid.
We will review your prescription information before we issue a refill.
If you are not taking your medication as prescribed, for instance, too many times or not enough times, we may question you further about your medication schedule. Medications are NOT prescribed over the phone or refills approved until a patient’s medical and medication history are available, therefore, no refills over the weekend permitted.
For safety reasons, we will only fill your prescriptions with the ONE pharmacy you designate, unless you notify us that you are changing to a different pharmacy. You must have only one designated pharmacy in your patient records with us.
Please be advised that we strictly adhere to the laws regarding drug administration.
We DO NOT prescribe OxyContin for any reason.
Please follow all instructions and precautions with your prescription medications that are provided to you by your pharmacy. If you experience difficulties with your medications in the form of side effects, contact us immediately to discuss the proper course of action.
For your safety, no refills can be given if you have not been seen by the Doctor within the last year. We will schedule an appointment for you to re-evaluate your conditions before refills can be given.
