New Patient Forms

We recently installed an Electronic Medical Record system to help us provide your care more efficiently.  
All of your medical information must be entered into the computer before your visit. 

To help us accomplish this, we have provided some pre-visit forms that you can complete ahead of time and bring with you to your visit.  Please complete and sign all forms as they instruct. 

This will make the check in and registration process easier and QUICKER for you!

List of all current medications and prescriptions
Your New Patient Information Form
Any Questions you may have for Dr. Sotos!
